Check in

Please arrive at our office at least 15 minutes prior to your appointment in order to complete your paperwork.

Payments and fees

For office visit, co-pays and deductibles will be due at the time of your visit. For surgery, you will receive an estimate for your co-pay and deductibles before surgery and payment will be due prior to or on the day of your surgery.

Cancellation policies and fees

Please give us 24 hours prior notice for any office visit cancellation and 7 days for cancellation of surgery. Office visit cancellations without prior notification will result in a $75 fee cancellation fee.

Form fees

We will be happy to assist you with your forms. A fee of $25 will be charged for each form.
