After making an appointment, each new patient will receive an email with a link to patient portal. The email will also include a username and temporary password. Use those to access the portal and click on the Forms tab to complete and submit your medical history questionnaire, demographics, and office policy and consent forms. Online submission of information via the portal is the preferred mode of submission, as it will allow seamless integration of your information with our electronic medical record system.
